Customized to Your Unique Requirements

First, we learn about your facility’s pre-admission process and information collection, document management, signoff, and scheduling requirements. Next, we help you select the health history and demographic questions (and any follow-up questions) Medical Passport will prompt patients to answer in the online questionnaire. We also load items such as your Centers for Medicaid & Medicare Services (CMS) documents and pre-op instructions into the system. Finally, we work with you to develop clinical forms or data export formats that provide information to the entire healthcare team. For example, registration staff will view or receive demographic information, anesthesiologists will access all medical data and health histories and can choose to automate their anesthesia guidelines. This results in efficient and secure sharing of data and gives each of your team members exactly what they need to do the best job possible.

Simple Systems Integration

Information from Medical Passport is available in either a printed format, or it can be interfaced automatically with your existing medical information system via an HL7 or XML interface. Our clinical forms can be customized to adapt to your current pre-admission process and smooth the transition to online data gathering. The system is set up to capture the information you need, and then to auto-populate items such as the pre-admit checklist, history and physical (H&P) forms, medication reconciliation etc. Medical Passport digitally updates documents with the patient or staff member’s name and a completion date and time stamp.

In addition, key aspects of the One Medical Passport implementation process are automated, which guarantees a uniform, rapid system implementation. Once you are live, your dedicated account manager will be your point of contact for any questions you have and will notify you when updates, upgrades and new tools become available.

Easy to Roll Out and Use

Your One Medical Passport account manager will work with you to set up your staff members and any personnel at the physicians’ offices you interact with as users in the system. Staff training will cover the system’s basic and optional tools you choose to use and the reports you want. Your account manager will also help you to roll out the system to your physicians’ offices with a quick online training program and will supply posters and signs directing patients online with a patient instruction handout. Physician office personnel may further be trained to upload patient documents, test results, consent forms, etc., to minimize faxing and filing paper documents. You may activate an optional automated phone service to remind patients who have not completed a Medical Passport to do so. This can significantly reduce the need for phone calls and dramatically improve patient compliance.
