Rapid Review of Patient Pre-admission Information

One Medical Passport’s Nursing Assessment view allows your staff to easily review and verify the information a patient submits in each Medical Passport. The information in this view is arranged into sections, such as medications, allergies, past medical history, etc., to simplify the pre-admission assessment. Nursing staff can quickly add additional required data or documents to the patient’s online file. The system maintains an audit trail that tracks exactly what information was changed, when it was changed, and who changed it.

Pre-admit Checklist Tracks Patient Status

This digital to-do list tracks the mandatory elements of the pre-admission process such as H&Ps, pre-op phone calls and surgical consents and allows staff to quickly see the status of each element. Each time a new piece of documentation is received or a new task is completed, the Pre-admit Checklist updates the patient’s status. This way, everyone knows exactly where the process stands and can focus on just the charts that need attention. Staff members no longer need to repeatedly flip through charts and affix sticky-note reminders.

Anesthesia Pre-Op Section Auto Populates with Verified Information

With the required information readily available in a patient’s Medical Passport, the system can easily generate a pre-op section customized to meet the needs of anesthesia providers. These providers can quickly review and update this section to streamline one of the most critical steps in ensuring that a patient’s procedure proceeds smoothly, safely and efficiently.

With increasing demands on anesthesia providers to cover more rooms, manage a patient’s entire perianesthesia care unit (PACU) stay, and provide complete perioperative analgesia, including nerve blocks, time for pre-admission assessment is at a premium. With Medical Passport, what used to be a lengthy patient pre-op interview is turned into a simple 1 or 2 minute review to confirm the information provided by a patient.

High-Risk Patient Screening Alerts

This feature cross-checks the information in each patient’s Medical Passport against your facility’s customized high-risk criteria. An algorithm helps anesthesiologists and surgeons determine the risk level for patients with comorbidities. It also helps them make special preparations for reducing complications in the operating room – preventing day-of-surgery delays or cancellations. Medical Passport “red-flags” patients with issues such as sleep apnea, increased body mass index (BMI), or cardiac problems that can be dealt with well in advance of surgery. In addition, our custom assessment tools for falls risk, sleep apnea, and deep vein thrombosis can calculate a risk factor based on patient responses to the Medical Passport questionnaire, and present this risk factor to healthcare providers.

H&P Automatically Populates

Medical Passport can populate the H&P form and present it for physician review and signoff. Electronic signatures can be used on the form, and completion of this critical item is automatically logged into the system.

Apply Your Pre-admission Diagnostic Testing Guidelines

One Medical Passport’s diagnostic testing criteria allow for the complete standardization of pre-admission testing guidelines. Currently, different surgeons, pre-op nurses, and anesthesiologists may have different criteria for ordering pre-op diagnostic tests. That’s why One Medical Passport uses your facility’s guidelines regarding which diagnostic tests should be ordered. For example, the guideline for ordering a blood sugar test might be any patient who is diabetic.  The guidelines for ordering an EKG could be highly complex: for example, all patients over 65 having major surgery, and all males over 50 who smoke and are hypertensive or diabetic.

Medical Passport’s easily configured pre-admission diagnostic testing guidelines replace the one-size-fits-all testing guidelines, and account for the specific surgery that is planned and a patient’s health history and specific responses to the Medical Passport questionnaire. The system’s easy-to-configure diagnostic testing criteria and templates can be easily modified as practices evolve.

Deliver Your Pre-Op Patient Instructions Online

One of the most important aspects of the pre-admission nursing process is giving the patient specific instructions on how to prepare for a procedure. This frequently includes guidance on NPO requirements, directions to the facility, and what to expect before, during and after the procedure. Written pre-op instructions are typically more thorough than those given by phone, and can be quickly delivered to and easily retained by patients. With One Medical Passport, your facility’s instructions are customizable down to the level of a particular surgeon or surgery. The system can automatically present the instructions to patients to review or print as soon as they complete their Medical Passport.

Centers for Medicare & Medicaid Services (CMS) Disclosures

Medical Passport is easily configured to include a compliance section that addresses both financial disclosures and a patient’s rights and responsibilities. This information is available to patients in written form and as an audio file that can be played in real time. The system then tracks each patient’s acknowledgement that they have understood the financial disclosures and their rights and responsibilities. One Medical Passport then stores this data in a full compliance report that staff can easily access.

Automated Reminder Messages

With One Medical Passport, you can set up reminder messages as automated calls or text messages to patients. This optional tool is simple to implement and control and improves compliance by reminding patients to create a Medical Passport.
